For your convenience the following forms can be found in the My CWA Customer Portal.
Application for New Service
For brand new CWA customers. If you are moving and already a CWA customer, then use the transfer service function.
Transfer Service
This is for existing CWA customers who are moving to a new address within the CWA service area. Use Transfer Service under Forms in the My CWA Portal.
Move Away
This is for CWA customers who are moving away from the CWA service area. Choose Stop Service under Forms in the My CWA Portal.
Name Change
To change your name, email, phone on your CWA account, choose Update Account Information under Forms in the My CWA Portal.
Billing Issue
Question about your bill? Submit for a review here, choose "Billing" as the Type of Inquiry.
Account Access
This is to give another person access to your CWA account, such as a spouse, child, tenant, etc.—Select "Guest Access" in the My CWA Portal.

Leak Adjustment Request Form
Leak Adjustment Procedure
- A leak adjustment is a one-time courtesy provided to our residential customers due to a leak past the water meter that has caused their water consumption to increase by a minimum of 50% over the historical usage for the same period. No Adjustment will be considered for an increase in consumption due to a hose bib that was left running.
- CWA will process and consider only one leak adjustment every three (3) years. The leak adjustment request must be submitted within (90) days of the date of the water bill in question. Only one billing period will be considered for a leak adjustment.
- Customer must submit completed form along with all of the receipts associated with the repair to: Chester Water Authority, Attn: Leak Adjustment Processing, P.O. Box 467, Chester, Pa. 19016-0467
Leak Adjustment Methodology
- The consumption portion of the water bill will be adjusted by charging the normal consumption at the regular step rate plus the additional consumption due to the leakage at the lowest step rate. Example: Normal consumption is 10,000 gallons but due to leak the consumption increased to 50,000 gallons for the billing period. Customer will be charged for the 10,000 gallons at the normal step rate and 40,000 gallons due to leakage at the lowest step rate plus the base charge.
Note: Adjustment will not be considered nor processed without proof of repair.
Property and Water Damage Claim Report
No action will be taken by Chester Water Authority until a written claim is received. Please complete this form in entirety and submit to this office as your claim. Incomplete information will delay the processing of your claim. You will receive a written acknowledgement of this claim upon receipt.

Water Certification Request Form
This form is used by Title Companies to request balances on properties prior to settlement. $15.00 fee per property can be paid by Check or Money Order. Payments should be mailed to:

Water Meter Installation Request
This form to be used by Developers/Contractors requesting installation of meters in new construction.
Before installation of meter, please be sure of the following:
- Customer’s service line has been installed
- Meter setter has been installed
- If required, install pressure reducing valve
- Address thermal expansion problem
It is necessary to provide advance notice for installation of water meters (10 Business Days)
